Please let us know a week in advance of your appointment if you have any special needs or require assistance.   

 

We invite you to openly discuss with us any questions regarding our services. We believe the best health services are based on friendly, mutual understanding between physician and patient.

 

Paperwork: For your first visit you will need to fill out paperwork online and submit prior to your appointment see New Patient Forms. If you wish a PDF version please contact the office.

Submit by email copies of pertinent medical reports, including X-ray, MRI or other imaging tests results, other test results, operative reports, etc.


Payment:  We only accept cash or check. We no longer accept credit cards.  


Cancellation Policy:  Kindly give us at least 48 weekday-hours notice (weekdays only, not including weekends) if you need to reschedule or cancel your appointment. Other patients are waiting for cancellations so disregard for this request will result in your missed appointment being charged to you. Exceptions may be made on occasion for special circumstances.

 

Strong fragrances: Please do not wear any strong fragrances, perfumes, essential oils, or engage in recent cigarette smoking, as other patients are sensitive and allergic to strong odors.


Clothing/attire:  We request that you wear comfortable, loose clothing for your appointments, as you stay fully clothed during treatment, except for your shoes. Please avoid wearing tight clothes, tight jeans, skirts, hooded tops, etc.

If you wish a PDF version please contact the office.  Printed version (PDF) will need to be filled out USING ONLY BLACK INK.  If using the PDF version of new patient forms you may print out, fill in, sign and bring with you or send to our office within requested time before first visit, for your first appointment.

5. Medical Records release form: this is for you to give your permission to have your medical records released between Dr. Spector’s office and another entity (e.g., another physician, attorney, etc.). Only use if needed.

Insurance & Payment

Our office is a Fee-For-Service practice and payment is due at the time of service.  We do not do any insurance billing directly. However, at the end of each visit, we provide you with a statement listing all the codes and information about your visit that you may send in to your insurance company for possible reimbursement. Please note that it is your responsibility to verify coverage and to submit any claims to your insurance carrier.


Since Dr. Spector is a licensed physician in the state of Colorado, most insurance plans can reimburse patients for visits. The amount varies with the health insurance plan. The only true exceptions are Medicare and HMO’s, both of which do NOT reimburse for Dr. Spector’s services.


Medicare: Dr. Spector is opted-out of the Medicare program, which is an option for all physicians. This means that Medicare will not cover or reimburse visits, and Dr. Spector and Medicare patients may not send in any billings to Medicare. However, Medicare patients are free to engage Dr. Spector’s services, and must sign an agreement at their first visit stating they understand Dr. Spector has elected to opt-out of the Medicare program, they agree not to bill or ask Dr. Spector to bill Medicare for her services, and they agree to be personally, financially liable for all charges. Some Medicare supplemental insurance plans may or may not pay for services not paid for by Medicare. This varies with the individual supplemental plan, and if you choose to attempt reimbursement from your Medicare supplemental plan, you must make them aware of the fact that Dr. Spector is opted-out of Medicare.


HMO’s: Dr. Spector does not participate with any HMO Plans, so they do not reimburse for Dr. Spector’s services.


GAP Network Exception:  A GAP Network Exception (also referred to as a network deficiency, GAP Waiver, in-for-out, etc) is a tool health insurance companies use to compensate for gaps in their network of contracted health care providers. It is a request to honor a patient's in-network benefits, even though they are seeing an out-of-network provider. Dr. Spector is considered by some insurance company policies to be an “out-of-network” provider, so they may deny reimbursing you at the same rate and/or with the same deductible, copay, etc. they use for in-network providers. Because there are likely no other providers in their network that offer osteopathic manipulative treatment within so many miles of where you live (this varies with the company’s policy), and/or no in-network physician who provides the same service Dr. Spector provides, you can ask your insurance company about making a “GAP Network Exception”, and honoring your in-network benefits to cover your visits to Dr. Spector. You would need to request this from your insurance company (most companies won’t tell you about it unless you ask).
